The deadline to file a Claim Form is July 16, 2026. To complete the Clam Form online, your Unique Notice ID and Pin Number is required to access the form. The Unique Notice ID and Pin Number are located on the Postcard Notice you received in the mail.
If you do not have a Unique Notice ID and Pin Number and believe that you are a Settlement Class Member, contact the Settlement Administrator, RG/2 Claims Administration LLC at 1-866-742-4955.
